Synapticure was selected by Medicare (CMS) as an Established Program for the GUIDE model to support families living with Dementia. This enables us to offer GUIDE services in all 50 states, we will begin supporting GUIDE beneficiaries in July 2024.
The Guiding an Improved Dementia Experience (GUIDE) program offers comprehensive care coordination for patients and caregivers living with Dementia, as well as access to in-home or facility-based respite care. All of this is available at no cost to beneficiaries.

For most ACOs and health systems, more than 10% of their Medicare beneficiaries have Dementia, and they often carry a disproportionate total cost of care (~$2,000-3,000 PMPM). Many of these patients may be eligible for GUIDE.
As an Established Model with national coverage, Synapticure is equipped to improve clinical and financial outcomes, collaborating closely with PCPs and referring providers to co-manage patients.
Synapticure is partnering with ACOs, health systems and provider groups in two ways:
Extend and enable provider networks by enrolling beneficiaries in our program
Helping other GUIDE organizations to satisfy all 9 care model requirements, including provider capacity to support the diagnostic workflow
Expert Neurologists and Psychiatrists available within 1 week from referral in all 50 states
Ensure exceptional patient and clinician experiences
Multimodal outreach to educate beneficiaries and support enrollment
Local services and SDOH coordination, disease education, and caregiver support
Ensure seamless sharing of visit notes, care plans and provider collaboration across EHRs and systems
Multidisciplinary model accelerates diagnosis, improves quality of life, reduces unnecessary acute care use, and enables beneficiaries to remain at home
