Despite this, too many people living with PD still struggle to access care, testing, treatments and clinical trials. It’s time to leverage the advances in science and technology to make medical care proactive and personalized for all people living with PD.
We built Synapticure to provide personalized care to people living with PD in the comfort of their own home. This means developing a personalized care plan for each patient through cutting edge testing and consultations with specialists at Synapticure and, if it is needed, movement disorders clinics.
This is how Synapticure works.
Synapticure uses telemedicine to make life easier for people living with PD. The stresses of the pandemic only add to the burden of traveling to an in-person clinic visit with a disease that can affect your ability to walk, talk and travel. Certain elements of PD care need to be handled in person and that is why Synapticure will always partner with a doctor or clinic that you see in person. That said, recent advances in technology and science have made it possible for much of your care to be handled remotely from the comfort of your home. Synapticure’s team of neurologists come from top movement disorders clinical training programs and are available when you need them for telemedicine appointments. Moreover, our doctors partner closely with the rest of your care team - whether that’s a local PCP, neurologist or PD specialist - to make sure our care is well-coordinated with them and that we’re all on the same page. And if you are not seeing a PD center of excellence and want to, or want to supplement the care you’re currently receiving, we will work with our network of partners to get you seen as quickly as possible.
In addition, from the moment you enroll, you will be paired with an experienced Synapticure Care Coordinator who will help you navigate every step of your journey and every facet of the care system. People living with PD often see multiple doctors and the administrative burden of coordinating across those care systems inappropriately falls on the patient and their families. Our care coordinators take this burden of paperwork and organizing specific PD resources off of you in order to truly provide a continuity of care. We bring all of your medical records together and organize them in a way that makes sure you and your care team have a clear view of your care goals, what is important to you, what you have been tested for, what medications you’ve tried, and what might be missing. In addition, our Care Coordinators provide you with support in advance of, and in between appointments, so you never feel alone. We will do the homework with you so you always feel prepared.
With Synapticure, every person living with PD can have the opportunity to determine if there are any known genetic causes for their PD. Our care coordinators work with you, our genetic counselors and our partner genetic testing labs to seamlessly and quickly get you the testing and pre- and post-test counseling you need to understand if you have a genetic subtype of PD. Even if you don’t have a known genetic mutation, Synapticure and our partners will periodically reevaluate your genetic test results alongside your medical history to provide you with updated clinical trials and other treatment options. Synapticure patients with other diseases such as ALS have already found previously missed genetic variants as a result of our care program that made them eligible for a currently enrolling clinical trial.
PD is an extremely heterogeneous disease. Every year we are learning more and discovering new genes and pathways linked to PD that could be more effectively addressed with targeted therapies.
