Guiding an Improved Dementia Experience (GUIDE) is a Medicare program that focuses on improving the quality of life of patients living with dementia and their caregivers at no additional cost. This initiative focuses on dementia care management to reduce strain on unpaid caregivers and enable people living with dementia to safely remain in their homes and communities.

Enrolling in the GUIDE Model through Synapticure offers many benefits, including:

  • 24/7 Phone Line: Access to a helpline with triage support available at any time.
  • Respite Care (if eligible): Some covered hours of respite care are provided by Medicare each year, providing temporary relief to unpaid caregivers.
  • Care Coordination: Help you manage care, schedule appointments, access services, and navigate home-based or community support for you and your caregivers.

A caregiver is any individual who provides regular assistance to a person with dementia, Alzheimer's, or any other condition. This can include family members or friends who help with daily activities, medical tasks, and emotional support.

Caregivers and family members benefit from the GUIDE Model in several ways:

  • Respite Services (if eligible): Temporary relief options that allow caregivers to rest and recharge while ensuring continuous care for their loved ones.
  • 24/7 Helpline: A dedicated helpline available for any questions or concerns, offering round-the-clock support and guidance.
  • Comprehensive Support: Coordinated care involving specialists and healthcare providers, including mental health professionals, to support the caregiver’s role.
  • Care Navigation: Help from care navigators to arrange local services such as meal delivery, transportation, and community support.
  • Educational Resources: Access to educational classes that provide valuable knowledge and skills for effective caregiving.

For eligible beneficiaries, respite care provides short-term relief for unpaid primary caregivers, giving them time to rest and recharge. This service allows a trained professional to care for your loved one at home or in a facility for a few hours or several days a week, ensuring continuous support.

If the covered hours of respite care are exceeded, you can continue using respite services at local rates. The  respite service is based on geographic location and available partnerships.

GUIDE is a Medicare benefit. Beneficiaries with traditional Medicare (not Medicare Advantage) and a  dementia diagnosis may be eligible for participation in GUIDE. If you are eligible to participate in GUIDE, you can receive access to these services at no additional cost.

If you do not have traditional Medicare, you may be eligible for other services that Synapticure can provide. We are in-network with major insurers, including Medicare and Medicaid.

To enroll in the GUIDE Model, book a no-cost intake with Synapticure. During this call, your care team will assess your eligibility for GUIDE. If you are not eligible, Synapticure may still be able to offer you additional services.

Synapticure collaborates with your existing doctors to ensure seamless management of your care. We work together to create and implement a personalized and comprehensive care plan that supports both you and your caregiver.

Your current provider remains your primary doctor. This service and our care team is in addition to the support you already receive.

Everyone from synapticure has been VERY helpful; from Eva and Dr. Jamie Hatcher. They are so kind and make you feel comfortable. They never rush you and they get things done so quickly.

Thank you for the access to genetic testing! It was discovered that I had a mutation of the SOD1 gene which has completely changed my treatment.

Patient 10

Extremely well organized and professional at each step of the process; intake, physician contact, follow up.

Patient 11

Great people all around. Taking great care of pALS and cALS. Predictable access and patient-centered support using a solid telehealth network.

Patient 12

Everyone is friendly and caring. The neurologist are great. They listen to what you have to say and help you anyway they can.

Patient 13

Synapticure itself is a Godsend. They have done more for me through telehealth than the clinic did in a year of in person visits. Synapticure takes care of all the legwork needed to do anything from getting a new powerchair to getting new medication. Actually prescribed me a medicine that I should have been on from my diagnosis. They are amazing.

Patient 14

I have a great experience with Synapticure. Michelle and Dr. Faber are wonderful.

We've struggled getting responses and support from other clinics about new drugs; meanwhile, Synapticure has been extremely responsive, and as a result, we have just started on Relyvrio. Additionally, Becky and the Synapticure team have been great! Thanks!

Doctor took her time with us and it was a most welcome change to what we have been accustomed to

Patient 5

The willingness of my neurologist to prescribe off label has given me new hope in my fight with ALS. It allows me to try something new while waiting for a clinical trial. For anyone fighting ALS , time is of the essence and the speed that Synapticure is something other providers should strive for. Thank you for giving me hope!
