The Centers for Medicare & Medicaid Innovation created the Guiding an Improved Dementia Experience (GUIDE) Model to improve quality of life for people with dementia, lessen the load on unpaid caregivers, and keep families in their homes as long as possible.  Through GUIDE, Synapticure offers comprehensive care coordination for patients and caregivers living with Dementia, as well as access to in-home or facility-based respite care. All of this is available at no cost to Medicare beneficiaries.

CMS recently posted our 2025 GUIDE Model outcomes — and we're proud to share what our team accomplished last year.
