CMS Dementia GUIDE

After years of advocacy for patients and caregivers living with Alzheimer’s and other Dementias, the Center for Medicare and Medicaid Innovation (CMMI) recently announced a new program to provide support for beneficiaries with Traditional Medicare.

As an organization founded by a patient and caregiver living with ALS, we couldn’t be more excited to bring this new model to families and caregivers.

What is GUIDE?

The Guiding an Improved Dementia Experience (GUIDE) program offers comprehensive support that improves the quality of life for people living with Dementia while easing the burden on caregivers. The program will launch in July 2024 for established programs that have demonstrated expertise and capabilities, like Synapticure.

Benefits for families

  • 24/7 access to caregiver support including a personal care navigator, skills training, disease education and support groups 
  • Care coordination including care planning, referral coordination and other services 
  • Access to $2,500 for respite care for caregivers—with no cost sharing—at home, at an adult day center or a 24-hour care facility

Eligibility requirements

  • Beneficiaries with Traditional Medicare (not Medicare Advantage) 
  • Has a Dementia diagnosis 
  • Living at home or in the community (not in long term care) 
  • Not currently participating in Hospice or a PACE organization
