After years of advocacy for patients and caregivers living with Alzheimer’s and other Dementias, the Center for Medicare and Medicaid Innovation (CMMI) recently announced a new program to provide support for beneficiaries with Traditional Medicare.
As an organization founded by a patient and caregiver living with ALS, we couldn’t be more excited to bring this new model to families and caregivers.
The Guiding an Improved Dementia Experience (GUIDE) program offers comprehensive support that improves the quality of life for people living with Dementia while easing the burden on caregivers. The program will launch in July 2024 for established programs that have demonstrated expertise and capabilities, like Synapticure.

We help provider organizations and ACOs meet care and reporting requirements, letting PCPs focus on patient care
Neurologists readily available to assess progression, manage medications, and make therapeutic recommendations
Navigators assist with telehealth and coordinate partnerships for comprehensive care of patients and families
Neurologists available within a week in all 50 states, with an NPS of 96; integrated behavioral health for patients and families
Partnership model and technology solution enables practices to easily refer patients, know they received care, and share notes and care plans
Beneficiaries with Alzheimer's and other Dementias typically represent 10-15% of patients, and carry a disproportionate total cost of care (usually $2,000-$3,000 PMPM of medical expense)
